I generally used this type of combat when it came to animals, like wolves,
tigers, and such.  Animals such as these try to knock down the prey and then
rake the target.  The characters feared when they went HTH with these
animals.  Also PCs would use this tactic when fighting a better and better
armed NPC.  I never saw any problem with the rule.
