I actually had a dungeon, in a Convention tourney.

HUGE bolted door, saying "DO NOT OPEN.  HUNGRY DRAGON!"

scortch marks all around the edges of the door, soot on the floor, bones,
etc.

there was a little window, if you listened you heard very slow deep breathing.
and could just barely make out some bones......

this was a common sense test.  would you open that door????
