I would say it means Point to Point.  I like to see the hexes as an easy way to handle facing for a map of interlocking circles of influence.  If you draw a circle around each hex with the diameter being point to point, then the circles interlock along the sides.  This accounts for "engagement" when you are standing next to someone.  You're both contending for the same ground.  
 
So, I think of it as a circle and the hex is just a guide for facing, limiting you to 6 directions.

I should already know this, but I'm having trouble finding the answer. The basic melee hex is described as a 1 and 1/3 meter hex. Does that mean that the hex is one and a third meter point to point, side to side, or is that the length of each side of the hex?
