(TFT) OOPS!!!

I once remember seeing an article (somwhere) which included a table of 
different "bad" things that could happen to wizards who rolled an 18 
attempting to cast a spell. A range of circumstances were listed from 
hilarious (the character loses his voice for the day) to apocalyptic (you 
accidently summon a demon...and he is really pissed).
Does anybody have a copy of this chart or have something similar that you 
use in your campaigns. I have three wizards in my campaign and I have a 
feeling one of them is going to mess up a die roll some day.
