Yes, though it's fairly modern. David Drake's Lord of the Isles series
(which ins't all that good otherwise), harps fairly constantly on spell
failure or unexpected effects. The one 'good' magician specifically limits
what she tries to accomplish with her magic to avoid failures or other
problems. Others try for more powerful effects, but it usually has a
downside.
