* You listed mundane equipment loss and magic item loss/damage but not damage/loss of more important mundane equipment like weapons and armor.

* Death/injury to animals, or non-PC casualties that are important to the PC.

* Getting sick.

* Situations which cause problems, like bad reputation, loss of rank, creation of enemies, reduction in morale, potential court martial, duels, desertions, criminal charges, transfer to other units, etc.

I think many players would have much more fun and satisfaction if any such fates get introduced in a way that gives them a chance to choose one of various responses and/or use their abilities to affect the actual outcome.

For example, they have orders not to split their force, but the enemy is attacking in multiple places at once, and they need to decide where to respond - the supply wagon, the animals, the picket guards, the commander's tent, the wizard's tent, or the sleeping men in the company.
