Underfoot doesn't say they don't get pushed back before going underfoot, necessarily. I think AM28 just means the trampler needs to be on top of the victim to trample. So moving onto them makes them make a DX roll and in either case they go back a hex, and in order to trample, the larger figure needs to move another hex to come on top of the trample target.
