HTH initiation is ambiguous, and it's been discussed on the wiki site:

I've played a lot of rounds in Death Test (solo) with HTH (there's a
gargoyle who will try HTH if I recall), but I think your interpretation is
right. I explain after your context:

I assume the Advanced Melee rules are used and that HTH can be initiated
either during movement (see wiki) or as an action.

In your case, I'm not sure you're right because "disengage comes BEFORE the
attack" (that round), but rather that initiating HTH *is* the attack (that
round). See Advanced Melee, end of first paragraph of Hand-to-hand combat
section, p15, "Initiating HTH combat is considered an attack."

Lizard man's "attack" that round is his HTH attempt (during
movement). Don't forget the defender's chance to prevent HTH initiation by
rolling a 5 or 6 when it's attempted. If he instead rolls 1,2,3,4, the HTH
defender would get an opportunity to try to disengage that same round,
provided HTH was initiated on movement (which assumes defender didn't
already act that round).

I think a special (somewhat moot) case happens if your lizard man picks
option II.e (advanced melee) where the HTH attempted initiation occurs as
an action (at adjDX time). In this case, if the defender already took an
action, he could not attempt disengage again this round, but he could next
round (and probably before the lizard man's HTH attack, assuming the
defender has a higher adjDX).

Also, 1/3 of the time HTH initiation will fail (defense roll of 5,6), and
if it doesn't, there's a 4d6 vs adjDX the defender can get out of it before
he's attacked (an HTH attack). Since disengaging moves him to an adjacent
hex, the weapon/shield he dropped is in the hex where the HTH initiator
jumped him. He had to drop it (there) when he defense-rolled 1,2,3,4.

HTH rules would have been much clearer with examples, including special
cases.
