Maybe represent improvised attacks  with clothing, dirt, shields &c with unarmed damage?

It depends how you envisage the creature using its insubstantial mode. Is it a method of escape, of evading pursuit, of penetrating an obstacle, of regenerating damage, what?

I generally dislike monsters that can make themselves completely invulnerable unless you have some specific gadget. (Slimes are a problem in this respect, if you don't have fire.) A PC standing around doing nothing is a bored player. Let them do something, make some contribution, even if it's not very effective.

Good question. It would be unaffected, but if the fire goes 
out, it would stop regenerating.  I’ll revise my version to 
answer this.
