I don't have time at the moment to run the numbers, but I think that your percentages below are not taking into account the automatic miss rolls of 16, 17 and 18 on three dice. All iterations of three dice 16 and above must be counted as a fail.

adjDX of 12 = 74% chance of success; adjDX 13 = 84% and so on). >
