Not at all. The attacker can still go HtH if the defender is
backed up against a wall, prone, surrounded (no use spinning then)
or when the other side wins the dice-off for initiative and makes
the defender move first, thus "locking in" his non-engaging hexes.
This seems to work well for animals--they try to knock the
defender prone, then go in for HtH.
We do HtH attempts as an action in my game and there's been no
great shortage of HtH combat. (And hardly any of it between the
players themselves!)
